ALCiS Health Testimonial Release
I give ALCiS Health (ALCiS) permission to use my name, city and testimonial or endorsement comments
(collectively referred to as "testimonial") on its website or in its marketing materials.
Permission is granted to use all or any part of my testimonial and to edit the testimonial for
length as ALCiS thinks necessary, provided that such editing does not materially change the information
or viewpoint I have provided.
I certify that my testimonial is true and accurate and reflects my honest belief in the quality of the
ALCiS product(s). I have no affiliation or connection with any person or entity connected with ALCiS and
have not been paid any monetary sum for my testimonial.
I waive any right that I may have to inspect and approve the finished website posting or marketing
materials, or the use of my testimonial therein. If I wish to have my testimonial removed from the
website or not used in future marketing materials produced, I may do so by contacting marketing@alcis.com.
